Volunteer Forms

Here at Habitat for Humanity of Benton County it is required that each volunteer fill out three forms prior to volunteering with us. The forms are a volunteer release and waiver of liability form, confidentiality agreement and publicity release form. If you are under 18 years of age, please fill out the minor release and waiver of liability form, and have a parent or guardian sign it as well.

Please fill out all required forms before you arrive for your scheduled volunteer event. If you have trouble filling out the forms online, please print off the PDF copies linked below and turn the forms in to the community engagement coordinator or point of contact upon arrival at the event site.

Volunteers are only required to submit forms once per year.
